Sans is bleeding because he has a bottle of ketchup inside his jacket, and when you kill him. The bottle of ketchup breaks and spreads around his body. While also dying. Making it seem he's bleeding.
In my mind, sans and papyrus came from a different universe, as they saw green grass, and there's very little green grass in the underground, and they didnt come from above, because sans' dialogue in the sans fight shows that he isnt if you look into it. The reason papyrus turned to dust is because he believed in you too much to continue to live, because he knew thats what you wanted.
