This isnt based on the main manga but instead the web novel. its been changed to suit the anime significantly as they get wet from snow not a lake or river. its explained that they are so soaked through that rudy can see through her transparent underwear. at this point she is so beaten up, so broken and injured shes got broken bones.
but beyond that she dosent try to cover up in the LN, infact she lets rudy look at her while he heals her. Either because she is an adventurer who is used to having someone heal her or because its a reward for saving her. Rudy never really knows why he just speculates.
something the anime changes a lot is the way healing magic works. you have to have physical contact with a wound or injury in the lore of mushoku tensei in order to perform healing magic which is why she is naked in the first place. Rudy not only needs to see what he is healing but also have skin on skin contact. her leg is pretty fucked up in the LN which is why he has to take off her shorts and other clothes for the other injuries.
